Below you will find helpful information about parking, insurance, appointment cancellation, and important things to bring with you for your appointment.

Important Things To Bring To Your Appointment

We ask that please bring these necessary items to your appointment:

Your Current Insurance Card(s) and photo ID:
We must have current information before you are seen. If you arrive without a current insurance card, we will need to reschedule your appointment.
Your Co-pay:
We accept cash, check, money order and Visa, Master Card or Discover cards.
Completed Patient Paperwork:*
You may access and complete these forms within the patient portal or here. If you prefer to download, print and complete, please print single sided only.Thank you!
*If you are unable to complete your paperwork before your appointment, we ask that you please arrive 20 minutes prior to your scheduled appointment.

Insurance Information

Our providers are contracted with most major insurance companies. However, insurance policies and guidelines are always changing and vary from plan to plan. We do our best to verify your eligibility before your visit but sometimes we are unable to gather that information or are given incomplete/inaccurate information. If the provider is not in-network under your plan, you may be responsible for payment of a significant portion, if not all, of your visit’s charges. To avoid this situation, before your visit-- we ask that you please check with your insurance company to be sure:

  • Dr. Kung, Dr. Collins, Dr. Lind, Dr. Suzuki or Michele Cathey-Hamley, FNP is listed with your plan as an in-network provider.
  • Your plan covers the services you need.
  • If needed, you have the necessary referral from your primary care physician.
  • For patients covered by the Oregon Health Plan or Health Share, we ask that you also know which managed care plan (Care OR, Family Care, etc) you are covered by before arriving. Please make sure that the doctor you are seeing is in that network and/or you have the needed referral or authorization.
Please note: For outside diagnostic services (such as labs and ultrasounds), we suggest you check with your insurance carrier regarding benefits and in-network facilities. When speaking with insurance representatives, you may need coding information which we are happy to provide.

Canceling or Rescheduling Your Appointment

If, for any reason, you are unable to keep your appointment and need to reschedule, please notify us. We appreciate 24 hours notice. We understand that unexpected things come up.

If you do not show up for your appointment and did not call beforehand to cancel —after two No Show appointments, we will charge your account $50.

Medical Records

A signed record release is necessary for release of your medical records from our office to another doctor’s office, a hospital or to you. Please download, complete, sign, date and return to us by fax or mail.

  • Cascade Women’s Health, PC record release form (Dr. Kung, Dr. Suzuki, Dr. Collins, Dr. Lind, Dr. Riley and Michele Cathey-Hamley, FNP).

If you are being referred by another provider to our office it is important that your records from their office be available for the provider to review before your scheduled visit. Please allow at least a couple of days to get them from the referring doctor. Everyone tries to send out in a timely fashion but sometimes may be unable to send records at the last minute. Thank you.

Parking information

Free parking in the Parking Structure 2 may be difficult at certain times of the year–the Winter holiday season especially. You will need a validation sticker to exit the parking garage at the end of your appointment. Please ask us to validate your ticket before you leave.

If you are having trouble finding a spot, please know there is FREE valet parking in front of the Good Sam Hospital entrance.
